Wildlife threats

In-Game Hostile Anomalies

These threats are encountered during patient treatment rather than at check-in or CCTV screening. They are confirmed from gameplay footage, but not yet assigned a formal danger ranking on this tier list.

Bed Monster

Black figure with round red eyes under patient beds. Step in its red zone and it grabs you. Give it Maple Syrup to pacify it.

Stalker

Extremely tall black bunny with a toothy smile. Looking directly at it costs 10 sanity. Avoid eye contact when passing.

Don't Look Up

Mass of eyeballs and flesh on the ceiling in patient rooms or the CCTV room. Looking up drains sanity rapidly.

Wall Anomalies

Groups of 2–5 figures that chase you if approached directly. Deal sanity damage and can steal items from your inventory.

Skin Walkers

Created when infected patients slip through check-in. Sharp teeth and wide mouths. Attack players and patients. Press E to break free if grabbed.

Headbanger

Patient at the check-in window smashing their head on the glass. Give it Coffee to neutralize. Ignoring it cracks the window.

Surgery Tentacles

Purple tentacles erupt in Room 8 with a 45-second surgery timer. Using a taser or gun instantly kills the patient.

Hiders

Camouflaged monsters that hide against windows, curtains, and walls. Getting too close triggers an attack. Listen for loud breathing and scan for their eyes and grin. Usually appear in groups of three.
