The Hollow Black Eyes anomaly appears on a patient whose eye sockets are completely black and hollow, with a frowning or grimacing mouth. In some cases the patient appears normal in person but the anomaly is only visible through the CCTV feed, making camera checks essential.

How to Spot It

Look at the patient's face directly at the check-in window. If the eyes appear as completely black hollow sockets rather than normal animal eyes, reject immediately. If the patient looks normal in person, double-check the CCTV feed — this anomaly sometimes hides from direct view.

CCTV Priority

Always check the waiting-room camera feed before opening the shutter. Some anomalies (including this one) only reveal themselves through the security camera, so a normal appearance at the window does not guarantee a safe patient.

Strategy

Build a habit of checking both the physical appearance AND the camera feed before accepting any patient. This anomaly is often a trap for players who rely on visual checks only.
