Barney is a white rabbit with black slit eyes, a mustache, a brown hat, and brown checkered pants who visits the hospital across multiple shifts. Each visit advances his quest chain. Helping him through all steps rewards you with an extra coffee machine (with a longer cooldown on the second use).

Quest Walkthrough
Visit 1 — Coffee
Barney appears at the check-in window and asks for coffee. Give it to him.
Visit 2 — Coffee Again
He asks for another coffee. You have about 120 seconds to serve him. If you close the shutter or run out the timer, he threatens: "Watch out. I don't like being refused."
Visit 3 — Red Briefcase
Barney asks you to keep his red briefcase safe. Accept it.
Visit 4 — Coffee
Another coffee request.
Visit 5 — Photo
Barney asks you to take a photo of him. Do so.
Officer Docman Appears
Officer Docman arrives at the hospital. He has somehow obtained the photo you took. Speak to him for story context — he's tracking Barney down.
Visit 6 — Coffee
Standard coffee request.
Visit 7 — Scalpel
Barney asks for a scalpel. This is the last time you see him in his normal form.
Visit 8 — Hide Request
Barney asks to hide inside your office because officers are searching for him. Closing the shutter does not refuse this request — you must let him in or shut him out manually.
Final Visit — Help or Refuse
Officers are closing in. You have a choice:
- Help him hide: Let Barney into the office. After the police leave, he rewards you with an extra coffee machine (longer cooldown on the second use).
- Refuse: Close the shutter on him. He leaves on his own. No reward.
Anomaly Form
When Barney is threatened, his normal form drops and he reveals his anomaly form: sharp teeth and a kitchen knife. He can kill if provoked — notably incapacitating Officer Docman in one known outcome.
Strategy Tips
- Always keep coffee ready during shifts where Barney is expected.
- The scalpel requested in Visit 7 is a normal treatment item — you can keep one in inventory.
- In co-op, assign one player to manage Barney interactions while others handle screening and treatment.
